Writing a formal letter

Hey all,

I found out that an internal medicine physician has his wife doing his billing and she is now looking for help. So I was planning to write a formal letter and then follow up with a phone call; however, how would I write it.

Would:

Dear Ms. ****
Dear Mrs. ****
Dear Dr. ** and Ms. **
Dear Dr. ** and Mrs. **

I remember that using Ms. Is a formal way to write a letter to a female even if she is married. It's just more professional. I would like clarification please :D

If you know her name, then I would use Ms. ----    If you do not, I would address to the office and attention to the office manager and biller. You have the right idea though by NOT trying to go around her :)
