5010 Patient signature

Hi all!! I'm hoping to get some help with some questions I have.

With the HIPAA 5010, do we need to get  our patients to sign new HIPAA forms? I've been researching, but haven't had any luck. One of our patients is a medical biller and she also mentioned that we also need a 2012 date in box 12 on the HICF? Is that correct?

Any answers/comments/suggestions is greatly appreciated! THANKS!!!

I have all my practices get one every year anyway. We also have them update their PI sheets and sign a new office/financial policy as well. For regular patient's they get sent these in the mail and asked to bring them in for their next appointment. We start mailing them at the end of Nov, every year.  CYA :)
