billing question need help

I have never exp this type of billing.  An out patient substance abuse treatment center, that transports patients to a doctor that performs medical exam, ekg, physical etc.  The doc charges the facility for his services however he performs them at his clinic.  My client wants me to bill for the services performed to the patients insurance.  She assures me that the doc is not submitting a claim for it.  How do i approach this and I thought i could not bill becuz services were not performed at my clients facility.  I want to do the right thing legally.

The dr would have to be linked to the facility in order to bill for his services I would think.  Since this is professional billing it would be done on the CMS 1500 form and the individual providers NPI should be in 24J, the treatment centers info would be in box 33 and the dr's office info would be in 32.  But if the dr is not linked, it may not pay to the facility.
