Block 32 Home visit

Hi,

Physician saw patient in the patient's home and I billed Medicare with pos 12.  Block 32 Had the physician's address.  Medicare denied stating incomplete info on where services were furnished.  When I called them they told me that I had to put the patient's address on box 32.  Has anyone ever heard of this.  I don't think there is anyway to put the patient's address in this field?

I have not heard of that.  I have always used the physician's office address in box 32 but I haven't billed any home visits recently.

Anyone else??   ???

If you bill for a home visit (12) you DO NOT complete box 32. 
Box 32 is only completed if patient was seen in an Assisted Living Facility (ALF)
Place of Service Code 33 or another type of facility.  At least that is how it
is done in Florida.

Forgot to add - if patient was seen in facility Other than home such as ALF Z(13) , then you put that address in box 32.  Hope this helps :)

Box 32 not filled out if home visit (12).

Yes, I did bill with pos 12 and Medicare denied it stating that they needed the patient's address on block 33.  I found that weird since they paid other claims just fine.

I think he meant 32...and I have had the same issue with Medicare denying for home visits my physician made. I had to state the clients address on the claim
