Homeless Person

Hi,

I know his might sound strange but what do I put on the form for an address for a homeless person?  I have to put something to process the claim electronically.  Anyone know??

What form?  I had this come up once but of course there was no claim to generate so I just used the practice address and made notation that there was no address. You can't submit to any insurance carrier without an address so I'm thinking maybe you are speaking of the PI sheet?  If so I would just have them leave it blank, just be sure you get the proper signatures.   If you are in need of an address, I would use the address on the patient's ID. Without proper ID in the first place it's not a great idea outside the ER to treat any patient unless it's an emergency, usually physicians should refer the patient to the ER.

It is for the cms 1500 form.  The pt has insurance but my client says the pt is homeless.  It is for an office visit for mental health.  Thank you very much for your imput!

You will have to have "An" address, best to use the one from their ID / Drivers License. If they indeed have coverage the carrier will also have the address on file, I guess if you had to you could call them and explain situation and see how you should proceed, but as I mentioned before, it's never a good idea NOT to get a valid ID from any patient.
