CLIA Waiver

I have a client who only does blood draws (venipuncture, 36415) in office--she sends all labwork out to a lab.

She is filling out her CLIA Certificate of  Waiver application, but was told by a doctor that a blood draw would need the PPM certificate, not the CLIA waived cert.  This is contrary to what I have learned about CLIA.  Can any one verify this for me?

I am not familiar with the PPM certificate.  I thought the CLIA waiver would have been appropriate.
