: Doc Signature on Billing Log?

I want to include a statement signed by the docs on my billing log....what do y'all think of this?

"I certify that the services reported above were furnished as stated and I give permission to ABC Medical Billing to submit claims for these services to the appropriate insurance carriers. I cerify that I maintain the patient's/policyholder's signature authorizing  the submission of medical claims."   

It's covered in my contract, but I thought safe to also have on the logs each week?

the doc faxes to me with pt name, cpt, dx codes and amt paid...he does not use superbills...

Oh Ok, like a daysheet,   Sure I don't see why you couldn't. Just a note about this part:
I cerify that I maintain the patient's/policyholder's signature authorizing  the submission of medical claims."   
 

I require the patients authorization for assignment to go in my records as it is a source document, it's on our patient information forms, so I think I would want to have that if I were you as part of your record.
