Hi,
I've recently started billing for a CRNA with CRNA employees. She has a business name she goes by. It looks as though the NPI number she has been using on the 1500 form is an individual number. If she has a business name with employees does that constitute a group and would she need to have a group NPI number? Would that go in Box 33a with the individual employees' NPI going in Box J? Should the business name also go in box 31?

Hi Kelly,
If she has a business name that she uses for billing and a business tax ID, then she needs a group NPI number. The group NPI number would go in box 33a, the individual NPI in box J and the name of the person who performed the service in box 31.
