how many service dates can you put on one claim?
does the diagnosis have to be the same?

You can put 6 service lines and 4 diagnoses on a claim.  The diagnosis doesn't have to be the same for each service line but you can't fit more than 4 different ones on a form.  You must indicate which diagnosis goes with each service line.
