I have a question regarding one of the offices I bill for always bills a well exam and separate office visit on the same day is that legal ??

As long as the other is medically necessary to be provided in conjunction with the preventative.  make sure to attach e Mod- 25.  The medical necessity for the additional E/M must be well documented

Yes, and be sure to attach the V code to the prev visit and your other diagnoses to your ov  :)

I would find it odd if they did that on EVERY well visit though.  It does happen, but not on every patient that has a well visit.
