My chiro wants to create a new superbill/soap note form all in one.  He said he saw one before. That way he is not using man power to rewrite the visit from the soap note to the superbill to send to me for billing.  He said the top portion had the soap notes and the bottom portion had the superbill.  Does anyone have a sample of this they could send me so that I can try to create one for my provider?  If not have they seen it and now where I can find it? I have been searching and have not found one.  Any help would be great.

Don,

    I haven't seen a superbill with the SOAP note on it.  It sounds like a great idea though.

It sounds like something proprietary to a specific software program.
