Do you have to include “that the patient was evaluated with the resident” in order to bill the daily hospital day charge?

Are you actually billing? Or are you completing some form of assignment? I'm just unclear as to the question. If you are billing for inpatient visits there is no place on a billing form to indicate "that the patient was evaluated with the resident". Can you give more details as to what you are trying to do?

I am billing inpatient pediatric daily charges. The resident writes the note. The attending physician does a teaching attestation. Does the attending physician have to indicate that he/she evaluated the patient with the resident in order to use the information that the resident wrote as part of the attending note? Thanks!

I'm sorry but that is out of my scope. That's not really a billing question as much as it is a procedural question. Who is on the bill as the rendering provider, the attending physician or the resident? I'm assuming the attending physician.
