If you are going from a single office to under a LLC do you get a New NPI or do you keep the same one you had before?
I thought once you have one that is your NPI for always but I have been told that I is not true.
Any help would be great.

When you say "going from a single office", do you mean the provider is currently registered as a sole proprietor and now wants to register as an LLC?

Yes
The office has three providers in it. They all have been under there own NPI, but would like to go to a LLC.
Don't they each keep there own and then add a group NPI?

They keep their individual NPI, but they now need a Type II, group NPI for the LLC.
