I'm looking for some advice on which form to use. We have a day treatment cognitive rehabiliation program for persons with brain injury. In the past we billed this by our agency statement to insurance companies, but are now being asked to use either UB92 or CMS1500 form with a CPT code.  The program would not be billed under any specific therapist provider. Any suggestions on which form and which CPT code to use? Thanks!

Hi,
   Are you billing for facility services?  (As opposed to physician's services)  It sounds like you are billing for the facility for the patients being there and not for any specific doctor's services.  If that is the case you would need to bill on  a UB04 (used to be UB92) form and you would need REV codes, not CPT codes.  These forms can seem quite complicated, but if you have some good instructions, it makes it much easier.  We do offer a book on How To Complete a UB04 Form.  For more information on that book visit http://www.medicalbillinglive.com/UB04-forms-book.shtml

Thanks for the info. Yes, that is what we are doing.. I thought the UB form was probably the correct one, but I really wasn't sure.  I am not familiar with a REV code though? Can you give me some idea of a resource? Thanks, this is really helpful.

The Rev code describes the facility service.  We included the list of Rev codes in our book.
