Hi Michele, or anyone,

Can someone tell me exactly what should be in CMS 1500 box 31? I have been sending claims with PT's signature to many insurance company and got paid.  Recently, he became participated with an insurance company but they are not paying. Over two months I called many times spoke to five representative they all told me claims are in process. Finally today someone told me that our claim could not be processed because PT's name should print (not signed) in box 31.  ???

I think the doctor's (or supplier's) "signature", tax ID & date go in box 31

Our box 31 is printed by the practice management system, never hand signed by the provider.
