Regarding Box 53 on the UB04 Form.  Your book states that for a "non-contracted" ASC you should enter a "NO" in the Assignment Box #53? Question? Can a Non-contracted facility "Accept Assignment"? I thought it was just to indicate that the patient agrees to have the benifits paid directly to the provider?, not that the provider agrees to "accept" whatever the payor pays?  So the question is: Do non-contracted ASC's always put "NO" in Box 53? 

I would appreciate everyones opinion.

Additional technical question?  Regarding "dates"? I have seen some sites that say dates should be "mmddyyyy" and your book indicates "mm/dd/yyyy" and even some sites say "mmddyy".  Question? What is the proper format or does it not matter?

A non contracted facility can accept assignment.  I reread that section of the book and I found it to be confusing.  Very sorry!  We are planning on revising the book soon and I will definitely clarify that box.  (Thanks for the insight!)  So in answer to your question, no you do not have to put no in Box 53.   :-[

I personally do mmddyyyy.  I no longer use the '/'.  I have a UB04 program that in some spots only allows us to enter mmddyy, but whenever possible I use mmddyyyy.
