facility type of bill

I have a few claims for United Healthcare Community Insurance, they are denying our claims, stating we are using an incorrect type of bill.  We use 831 on our claims, 2 particular claims they denied.  Wont give me any other information.  How does the type of bill change, and any ideas what I need to use.  Thanks

If its outpatient then it should be 131

Some insurance carriers will accept one type of bill, then change their system or policy and the type of bill changes with it.  It's not that uncommon.  Just a pain for the billers.  :)

I don't know if I missed something here but you didn't say what type of facility you are billing for so I'm not sure what might be wrong with the type of bill.
