Nerve conduction and EMG denials?

Has anybody noticed a pattern of denials or any particular pitfalls when billing for electrodiagnostic studies?  If so, what are the major problems with billing?  I thank you for your help in advance!
These are the codes commonly used:


95886
Needle electromyography, each extremity, with related paraspinal areas, when performed, done with nerve conduction, amplitude and latency/velocity study; complete, five or more muscles studied, innervated by three or more nerves or four or more spinal levels (List separately in addition to code for primary procedure)

95900
Nerve conduction, amplitude and latency/velocity study, each nerve; motor, without F-wave study

95903
Nerve conduction, amplitude and latency/velocity study, each nerve; motor, with F -wave study

95904
Nerve conduction, amplitude and latency/velocity study, each nerve; sensory

95934
H-reflex, amplitude and latency study; record gastrocnemius/soleus muscle

No.  No patern of denials.  ALso, no major problems.

Maybe verify that you have a diagnosis code to support the study?
