Hi guys!

I'll be getting my first client this week and I'm super excited.  The therapist that I'll be working with is receiving denied claims indicated "address not on file."  He recently changed locations/addresses.  We'll talk later this week but wanted to see if it's safe to assume that he hasn't given the insurance enough time to update his address AND if he has should I simply follow-up with the Provider Rep?  What would be my best course of action to get this resolved and the claims sent back for reprocessing?

OR, do you think I'm jumping ahead and I should simply wait until the therapist and I talk more this week?

I appreciate your feedback/assistance.

You want to send all the carriers with the old address an updated W9 along with a letter.This is the most efficient and sure fire way to make sure it's done. You will want to make sure all records (NPI, IRS) match exactly!

While most of the payers ask for a copy of W9 form along with a letter with the details including the effective date of the new address, some payers take these info only through their specific form which can be downloaded from their websites. They generally call it Demographics update form or Provider info update form, etc.  Some of them are online applications [to make our job easier  ;) ]. You should check with the payers' website to be on safer side.

Just in case it is applicable to you, therapists who also supply DME equipments will need to obtain a separate NPI for the 2nd location (if it is going to be an additional location). Good luck.
