Hi everyone, I am getting forms together during my marketing phase and I would like to know what forms are needed after signing the doctor. Obviously the contract but what other forms. I know I should get the NPI#, provider practice name, fax and phone #'s and legacy numbers but I feel that this is not enough and other forms are needed. What other forms are missing? ???

When we sign a provider we take:
- contract
- provider information sheet (to get all practice/provider info, address, EIN, NPI's, etc)
- any EDI paperwork for electronic claims
- contact info for our office (phone, email, name of person who they will deal with, office manager, etc)

Do I get the EDI paperwork from the EDI website? I may be signing up some providers who are not filing electronically so I am trying to be as best prepared as I can beforehand.

Most insurance carriers have their edi paperwork on their websites. Depending on your clearinghouse, you may get it from them.

Your software vendor should be able to help you with EDI paper work.

For Medicare Providers I always remind them they need to update the billing officer on their enrollment.
