One of our billing clients is a Hospital with all of the staff belonging to the hospital. With that both physician billing and facility billing occurs. At our Hospital, we have a lot of NPP staff that preform billable tasks (ranging from EKG to PT Tech) with the Supervising credentialed doctor overseeing the work and meeting all the regulatory requirements of Supervising. My question is this: On a CMS1500 do you bill as:
Supervising NPI - Box 24J
Supervising Name - Box 31
Facility Name and NPI - Box 32 and 33
Or
Supervising Provider - Box 17 with DQ qualifier and NPI in 17B
NPP - Box 31
Facility Name and NPI - Box 32 and 33
With box 24J completely blank as the NPP doesn't have NPI
