CMS 1500 FORM

Thanks for this wonderful forum.

In new ANSI 5010 setup, we need to enter Facility address both in block # 32 & 33 and because of this many secondary insurance are sending their payment to hospitals instead of Providers.

Please guide me how to solve this.

facility information goes in box 32 and billing provider goes in box 33

Hi Regina,

As per 5010 Update, we are suppose to use the same facility address in block # 32 & 33. We are not supposed to use our correspondence address (Billing Address) in Block # 33.

billing provider is still in box 33-this is where payment info goes.  i have had no interuptions from the transition to 5010.  this is where my info goes.  i am merely going by what i have been successful with.  i have been 5010 since march.

Only if the address is a PO box, perhaps that's the problem?

With the 5010 PO Boxes are no longer allowed in Box33

All our secondary payments are sent to Hospital address. Please guide how to resolve this :-\

If it's not a PO Box, I'm at a loss at what could be problem and I would contact your clearinghouse first.
