Hi Michele,
I was wondering I know now that when you bill for a optometrist you had said you bill the office visit to Medicare and the glasses to DMERC, but in my software there is not a option to choose this type of billing center. When you set up the patient you have to enter primary, secondary and ter. and then when you create a claim you choose from a drop down list of their ins co, I do not see a way to choose DMERC, do you have any tips on what you do in your software.

I know but in my software when you create a claim and you choose your carrier you choose from only the patient ins. carriers listed in a dropdowm menu that you have to enter when you create a new patient file and the choices are the primary, secondary, and tri. if only it had a option for say (other) where you can enter something like this. How does your software do this.

You should be able to load the carrier and then choose it.

We load the patient file with Medicare and the appropriate secondary info.  Then we enter the claim for the exam.  Then we start a new claim for the glasses and then just manually change the primary carrier from Medicare to DMERC just on that claim.  You should be able to access the insurance info and change it for just the one claim.

If that doesn't work, and you are submitting the claim on paper, you can leave it set up as Medicare and place a white mailing label with the correct info over the Medicare address.

Thanks everyone for all the advice, I guess your right Michele if I have no other option I'll have to send paper, I was thinking what about entering the patient a duplicate time and using DMERC as primary to send out this type of claim what do you think?

You should be able to load the insurance (Medicare-Dmerc) in the patient's account. This insurance should be listed under self pay so that it won't file in other cases. You shouldn't just "edit" the insurance for that one claim. When you send the claim for the glasses, you will move Dmerc to the primary position, and put Medicare under self pay. You should always be able to change the COB's on your accounts because insurance changes often.

let me ask you what software are you using, because I cannot find this option in mine it only lists primay, secondary and ter. the only way I can see is if I change the patients primary for this type of claim and then change it back after I generate the claim. This is crazy but I cannot find another way.

I use TDME and we can load multiple insurances. I also use Office Ally, and Misys. I can understand your dilema. Are you saying your system only allows you to load 3 payors??? Since you have to create a separate claim for the glasses, I would guess that you can only do as what Michelle stated. Edit the primary on the 2nd claim and load Medicare Dmerc into your system as primary. When you have to bill another visit, you will have to change it. I have also used IDX, and you can load multiple payors, and change the COB depending on the situation.

OK, never heard of them, so I wouldn't know my way around their system. I hope you can maneuver the claim the other way so you can send it electronic.

I guess all I can do is change the primary and send out the claim and then change it back.

You could do that, but I'd really be surprised if your software doesn't allow you to switch the carrier at a claim level.

I have to agree with Michelle, the ability to change the carrier on a claim level is a rather needed function in today's world due to all the changes.. I don't think I have worked on a PM software that you couldn't make that change on a claim by claim basis.

That would be difficult for me too Linda, I just don't know her system to help her see if its possible. Customer service should be able to assist.

I emailed to customer service and he was not sure what the other billers where doing in this case.
He did not say that there was a option, I think I will try to call him later today because I am wondering if he understands what I am asking.

If it were my software I would want to know if it's an "option" or a "work-around" and also look at what the standard is and find out why that feature is not standard in your software. There were many instances I needed to switch carriers on a claim by claim basis! the only work around I can see for a software that doesn't allow this is to delete one carrier, add the other and re-add it later on ..but that surely isn't productive or efficient.
