l3660 denial

Is anyone else getting l3660 and l3670 denied?

Maybe you would have better luck if you stated what the denial is.  It might not be code specific.

J65 - $35.00 THE QUANTITY OF UNITS BILLED EXCEEDS THE MEDICALLY UNLIKELY EDIT LIMIT.  This is only Cigna ins so far.

I'm thinking this is not a "coding" error but you need to look at policy and number of units billed. I would call them and get clarification and take it from there.

I agree, start by calling Cigna and asking them to explain the denial.  Especially if this is the only carrier that is giving you that denial.

Thanks, I did call Cigna and it looks like they are having issues on their end. Rep was able to look at the claim and send it back. To date we keep having the denials and having to call on each one. I have had only one claim process but it went to the pt ded.
