J0585 Botox for Migraines

We billed Medicare for J0585 qty 3.  2 were denied.  Can anyone tell me why & the guidelines please?

I can't believe they don't cover acupuncture OR cold laser therapy at this point.

I know right!  They cover hyponotherapy but not acupunture.
