Questions on filling out 1500 for workman's comp?

For a workman's comp claim, on the CMS 1500, where do I enter the claim number?  Does it go in 1a, where the patient's member ID would normally be entered, or in 11?

I am a provider in Michigan, and we had to let go of our biller, unfortunately.  I'll be submitting claims for a few weeks, while we find a replacement.

When we submit WC claims using CMS forms we put the claim number in 1a, or the insured id number spot.  We also put the WCB number (in NY all WC cases are assigned a WCB number) in the group number field.

I am also in NYS and beginning next year, licensed acupuncturists will be able to bill WC. I have not billed NYS WC in many years...but I recall that the lengthy C4 forms needed to be filled out for each dos...

is that still the case, or can we submit on CMS 1500 form now?

Yes, you can put it in 1a and some people practice it by using insurance ID field as well.

You can bill on CMS 1500 forms now, you just have to attach notes.  They are doing away with the C4.2

Is the C4 still required for initial visit?  and do we still have to fax a copy of each claim to the WCB?
