Physician purchased lab services

A physician I bill for wants to begin purchasing lab services and billing the global service to the insurance himself.  Is it correct that we have to report the amount paid for the purchased service on the claim?  Is he then only paid for the amount he paid, or does he collect the global fee?   He has been told he can make thousands of dollars by billing for the fees the lab normally collects.   Any input regarding this would be appreciated.

You do report this on the CMS form in box 20 I believe, but I am not familiar with what to report.  I was hoping someone else on the forum had experience with this.
