Anesthesia Billing

Hi there
I know I have posted about this before, but I keep getting rejections on the anesthesiologist claims for HCPCS for all my cataract DX's. The HCPCS we are using are 4255F, G9363,and G8427. I tried to find out on the Medicare website if any of these are not in use anymore and cannot seem to find out. I cannot call CARE because these claims have not made it to CARE yet they are rejecting from the clearinghouse. Can anyone shed any light? Thank you.

I thought that the anesthesiologist billed using the same code as the surgeon?  4255F is a PQRS code I believe.  G8427 is just indicating the dr documented the chart.  It looks like the codes are the issue, not the diagnosis.

Hi Michelle
We finally figured it out. It was the PQRI codes, we are only able to use the G8427 and none of the other ones for Medicare of Vermont. Isnt it funny how they all require different things? You would think it would just be uniformed? Thank you for replying to me on this.
