IUD Denial

We had a patient who had a hysteroscopy and an IUD insertion at an outpatient facility.  UHC paid for the hysteroscopy and the insertion but they are denying the actual IUD device which we provided. One time they said it needed a modifier and now they are saying it cannot be billed at an ambulatory place of service, which it can be!!  Any suggestions?

Have you tried filing an appeal?  Or is the information below from phone conversations? 

The modifier explanation seems off but it may be a plan limitation. 

Did you receive an eob with a denial?  If so, what was that reason?
