EOB STATES PT NOT RESPONSIBLE FOR CHARGES

I'm asking question for a colleague.  A commercial insurance carrier denies a claim and the EOB states that the patient is not responsible for charges.  Then how does the doctor get paid?  This is a service normally performed by the doctor, and insurance verification advised no authorization needed.  Can anyone offer any explanations?

There should be another remark on the EOB regarding the denial. Can't help w/out complete information. Billed Services, charges, and EOB reason for denial is needed to further advise you.
