I am a NP who wants to start my own business.  I all ready have an NPI number.  I wish to notify Medicare of this change and want to start billing them.  I also wish to start billing Medicaid.  What forms do I need to fill out with each agency to do this?

Medicare requires the appropriate CMS 855 form.  You can get them online via your Medicare carrier website.

You might want to check your State Medicaid vendor/carrier on what they require for enrollment.  Ech state is different. 

YOu might also want to obtain each entity's EDI agreement and see what they need for electonic claims submission.  You may have to send each test files equal to a file you would send for processing.  Once reviewed and approved, you would be moved from test to real claims mode in their system.
