Pain Management Billing

I recently went back to billing for the pain management Doctor in our practice, after a hiatus for about a year.  After inheriting back a crazy AR. find that Medicare is not paying for Bilateral 64493,64494. The charge posters have tried different modifiers and I have appealed .The same is going for 64483,64484, they cite that the frequency/ days or units of service exceed max.  Help!

How many units are you billing the date of service? You want to make sure that the insurance will allow for you to bill for all of the injections you are trying to bill for on a single date of service.
