A Client’s NPI Registry information shows:
Medicare UPIN   ******
Medicare ID – TYPE UNSPECIFIED   ******
Client wishes to be removed from all Medicare entanglements (panels, enrollment, etc.). Is this possible and how would one go about it? I have heard the phase many times, “You can’t opt out of Medicare”.  I’m not sure if this statement is applicable in this situation.

Removing the info from the NPI Registry won't remove the provider from Medicare.  You can voluntarily terminate your Medicare provider number by completing the correct CMS form to notify Medicare that you no longer wish to be a Medicare provider.

The following website has info on opting out of Medicare:

Chiropractors cannot "opt out"

A chiropractor, who realizes opting out of Medicare is not an option, thinks he can "not accept assignment". How would one go about helping this person? Would going through the steps outlined in the above link accomplish getting this provider into Medicare's preferred provider who does not accept assignment category?

Is he already a Medicare provider?  If not, he would enroll as a non participating Medicare provider.  If he is already enrolled in Medicare as a participating provider he will have to wait until the end of the year to notify Medicare he will be changing his participation status for 2011.

Yes. He is a current participating provider. I meant to use the word participating and not the word preferred.
