]I am new to medical billing. Really I am still studying. I will graduate this month. I have a question about the cms form, my teacher is learning with us, she doesn't exactly know how to fill one out correctly. I wanted to know how to correctly fill in the physician/supplier section? I am still a bit confused about the physicians info from Item 24-33. Can some one please be a REAL teacher and help out a student?? I really like medical billing but she was a cook before teaching, she knows nothing else about medical billing than what the book is showing her.!!!

Thank you SRINIVASUK, that is the best site I have seen. Very detailed instructions. I willl give it to my teacher so she doesn't waist time for the next batch of students!!! ;D   ;)

she shouldn't be teaching if she didn't know that. She could have gone on NUCC to get that same information.
