Question on box number 50 on the UB04 form.

Hello Alice I have another question about the UB04 claim.  In box number 50 our claims are printing with the insurance we are filing to first and it seems to switch them. 

Example:

If a patient has BCBS primary and Medicaid secondary. 

When I file the secondary claim to Medicaid, Medicaid is listed in the A slot and BCBS is in the B slot.

Is this correct?

Hi DeeAnne,

   Yes that is correct because it is printing the insurance that you are billing out to in A and then any other insurance (whether primary or not) in B and so forth.
