I've gone twice and won on both cases. The process does vary depending on your own small claims process for your state. I know that you cannot have an attorney represent you in small claims, but I would however make sure you have an attorney who will backup your contract. If you are in a state where it is illegal for providers to enter into any fee-splitting arrangements and you are charging a percentage of business, you would be better off taking other means to collect as your contract will be thrown out of small claims (null/void).
On another note.. Always use going to court as last resort. The two I took to court were a last resort. Normally doctors do NOT like to have their credit tarnished, try first regular collection action's before going to small claims. Also you need to make sure the amount you are trying to collect is not above the threshold amount in your state for small claims collection.
