Billing for an in-network exception

A patient of ours has insurance through a company we are not contracted with.  She asked for an in-network exception and was approved for one.  They gave her a billing code to give to us to bill with.  Where do I use this billing code?

what do you mean by billing code?  is it a cpt code or more of an authorization #?

If it is like an authorization number I would put it in box 23 on the CMS form (or the equivalent if submitting electronically).
