I just want see if anyone can confirm what I've been able to research online thus far. I am trying to verify the maximum number of dx pointers any line of service can have (i.e. in box 24E).

I've read that it should be a max of 4. Is that right?

The EHR we use has the option for 8 dx pointers and by default it is filling them all in (if the pt has 8 dx). When claims are generated there are lines of service with 8 dx pointers and I believe it is causing the error at our clearinghouse. So before I bark at the EHR developers for allowing 8 dx pointers I wanted to verify that it should only be 4.

The pointer gets set when you link a diagnosis code to the target procedure code.  Only those diagnosis codes which get linked to the target procedure code should be showing up in Box 24E.

If your software is automatically filling all 8 DX code slots, there must be a setting somewhere for you to change this behavior.  Either that, or the software is expecting you to delete the unneeded DXs.

Correct, up to 4 pointers. up to 12 dx codes (1-12)
