Fitting of Urinary Device

Would anyone know what to code for the CPT (99212) when billing for a fitting of a urinary device (v53.9)?  Can a nurse practitioner bill for this as well?  I have a doc that goes into SNF who receives cappitated pay from bcbs and he want to know if he could bill for fitting of a urinary device under an office visit code and be reimbursed out side of the cap?  Thanks for your help everyone.

I don't know if it is covered outside of the cap but you would use the dx for the reason for the urinary device.
