: Billing for NON-CLIA waived tests

I do drug screens (G0479) using a NON-CLIA waived test (Pictus 400 chemistry analyzer).  I can't seem to get paid on these.  Can someone help me go through the steps to make sure that I am not leaving something out that would cause my claims to not process?

This is not a CLIA waived test.  It is a high complexity test.

Is it one insurance carrier in particular or across the board?  Is the provider registered to perform these tests?  Does Medicare and/or other insurance carriers have the lab information on file?
