dr and pschologist billing in a treatment center

Ok this keeps evolving...it looks as though I will now be billing for the dr and therapists that are going to work in the treatment center - does that billing go on a ub04 or 1500? The license type is "day or night treatment with communtiy housing". What series of hcpcs codes would the treatment protocol and room and board fall under or is it a combined per diem code? It is a 28 day or less program. You have helped me so much already! Thank you! Jeannie

The professional fees usually get billed on CMS 1500 forms.  Room and board fees are usually billed as REV codes, not HCPCS.
