Hello,

I would like to know if anyone has a copy of a day sheet and HIPAA forms for doctors offices. I have a new practice not sure of any other form that are needed. Please advise!

When you say day sheet, do you mean superbill?

A day sheet can mean different things to different offices.  A day sheet in my office is the printout I do at the end of the day showing all charges entered and all payments entered.  My PM software creates this report.

Other offices call a daysheet the sign in sheet showing all the patients that were seen that day, it's matched against the PM software report to make sure all the patient's services were entered.

I would google. HIPPA Forms and your state. should give you more options than you care to have. I would use the forms online as a guide only so as to add your practice name and address and logo on your form. As for the day sheet you could A) prob purchase them, or B make up your own in Excel. making your own in Excel is cheaper...
