Provider or Practice Contract

Can anyone direct me or possible post a sample provider contract that covers all the bases?  Im having difficulty figuring out how and what to charge my provider and could use any assistance.

It is not good to use a "sample contract".  Contracts are very personal and must be tailored to your own business.  We used to have a very basic contract that didn't cover anything really.  We knew we needed something more complete but didn't know where to start.  So we actually wrote a book on how to write a contract and then used that to write our own.  I know I sound like a commercial and I'm being up front about the fact that we are selling a book, but I really would strongly recommend you do not use a sample contract.  The things you need may be very different than what another service needs, and you really don't know anything about that sample. 

There are several things you need to consider:

  • Payment - how it will be made to you and when it is do
  • what happens if they don't pay on time
  • how a contract is terminated
  • what the provider is responsible for
  • what you are responsible for
and more!

I used Michelle's book to help me come up with a rough draft.  I also incorporated some items that I have learned the hard way that I needed to be in my contract.  I took it to an attorney and had him put it into a legal contract.

Michelle's book on writing a contract was probably the single best thing that I purchased for my business! I read it cover to cover, highlighted, bookmarked etc. I then used the info to create a rough draft which I brought to an attorney who formed the contract based on my needs.

Worth every penny! :)

So glad to hear how much it helped!  That's exactly why we wrote it.  We wrote the book before we modified our contract and used the book ourselves!   ;D
