: E Book for UB04 form

I was lucky enough to find Alice a few years ago when I had questions about completing facility claims.  (at that time the form used was UB92) Since then, Alice and her staff have helped out many times.  Most recently, when facility claim forms were changed, downloading her E book for UB04 forms has been extremely helpful.  Since using the book, I have successfully completed many claims, and our facility has been paid with no problems.  Thank you Alice!! 
ps  the cost of this book is extremely reasonable too!!
