medicaid denial

I keep getting denials from medicaid for family planning. the codes are 88142/88141 with dx codes v25.09 and 616.10. But another denial was received stating (claim includes family planning diagnosis(es) and no family planning procedure. Please resumbit with family planning procedure/modifier or correct the diagnosis. So far what I can see these are the correct procedure given by the pathologist. What am I doing wrong. Can someone help me? Thanks.......

The 88142 & 88141 codes are pap codes, and the dx V25.09 is for family planning.  There are no family planning services being billed.  Why are they indicating the V25.09 diagnosis for pap codes?
