Hi,
   I'm not sure what link you're looking for but if you are billing for the 64614 and it is bilateral, then you would bill it on one line with the 50 modifier to indicate both.  Bill the amount for both sides.  Then you bill the 64613 with the 51 modifier and the number of units that were performed.
