: Billing for mobile NCV testing

We are starting a new NCV company and want to use trained techs to go to different physician offices to do NCV testing.  I need to know exactly what to put in boxes 24J, 25, 31, 32 a or b, and 33 a or b.  We are obtaining an EIN for the business name and an NPI number. Do we use the Business name for box 33 and box 25?  I am confused.  Thank you

24J  -  individual NPI of the rendering provider  (provider who performed the services)
25   -  EIN for the billing entity (business that is going to receive the $$)
31   -  name of the individual provider who performed the services (the provider that matches the NPI in box 24J)
32   -  name, address, and NPI of the physical location that services were rendered
33   -  name, address, and NPI of the entity (business that is going to receive the $$)

For box 31 and box 24J does the servicing provider need to be an individual physician?  Can we put the same information as in Box 33 - the name of the company or must this be an individual?

24J & 31 are supposed to be the actual provider who rendered the service.  It should be an individual person, not a group.
