co pay - facility or physician collected?

In the HBOPWC (Hospital Based Outpatient Wound Center - functions as an outpatient free-standing center) where the MDs are not employees of the hospital, who collects the co-pay? The MD providing the service or the facility? I have searched around for an answer but haven't found one. Thoughts and advice, or a site to look up the answer myself,  would be appreciated. Thank you.

If the MD's are billing separately, under their own EIN & NPI then they would collect the copay for the professional services.  If the patient has a copay for the facility portion of the claim, then the facility would have to collect that.  For example, if I go to ER I have a copay for the ER.  Then the physician bills my insurance separate from the hospital.  I owe them separately.
