Workers comp forms on CMS 1500s

I got this email this week and thought it might be of interest

Hello Alice,

I tried to search your website for an answer to this question, but couldn’t find one.

I have been in billing for about 10 years and I recently moved to another company.

In the past, when filing a workers comp claim on a 1500 form, the patient’s name went in Box 4 with their address in box 7. Employer was listed in Box 11 B.

This company files the claims with the Employer name in Box 4 – Box 7 is blank and Employer again in Box 11 B.

They have not have any claims returned, but I was just curious which way was the correct way, or does it matter?

Hi,

   We file almost all of our workers’ comp claims on workers’ comp forms but when we do file on a CMS form we bill with the patient’s name in box 4, address in box 7 and employer in box 11b.  I guess if they are getting paid the way they are doing it then it’s ok, but I haven’t heard of that way before!
