Question:  Can I bill 1 UB04 with charges for facility?   And can i bill another UB 04 for the mental health therapy?  I have always known that professional services are on the 1500 and facility on the UB04.   So if a pt is being seen at an out pt facility for counseling, can i also bill for facility fees or use for the facility even though its out pt.   My exp is we can bill only if its In patient such as Residential treatment for both.    Such as Residential:  Rev Code 0912, diagnosis 304.00 
            or Rev Code 906  for the Mental Health Counseling

Trying to figure out if its different for only Outpatient facilities.   Hope someone understands

I'm not really sure what your asking? Are you asking if you can bill both the professional charges and the facility charges on one? If so no, the professional services have to be billed on the CMS 1500
